I chose the article "Measuring the benefits of clinical decision support: return on investment".  I clicked on related links and several articles dealing with the cost and benefits of computer-based clinical decision support were provided.&lt;br /&gt;&lt;br /&gt;Google:  I searched the term "Computer-Based Clinical Decision Support System".  I received over 50,000 results.  I chose the first result.  The article "Evaluation of computer based clinical decision support system and risk chart for management of hypertension in primary care: randomised controlled trial " provided information about the effectiveness of computer-based CDSS using a randomised control trial in hypertension patients.&lt;br /&gt;  &lt;br /&gt;&lt;br /&gt;CCHS Library:  I searched the term "Computer-Based Clinical Decision Support System" and received no results.  I searched the term "computer" and also received no articles that appeared to be relevant.  I searched the term "support system" and also received no relevant articles. &lt;br /&gt;&lt;br /&gt;Search 2.  Integrating Research Evidence&lt;br /&gt;&lt;br /&gt;Pubmed: I searched the term  "Endarterectomy, Carotid"[MeSH] AND "Cerebrovascular Accident"[MeSH] AND "Carotid Stenosis"[MeSH].  I used the limits  English, Randomized Controlled Trial, Human.  This returned 17 articles.  I chose the article "Risk, causes, and prevention of ischaemic stroke in elderly patients with symptomatic internal-carotid-artery stenosis".  I then clicked on related links and received several articles on the use of carotid endarterectomy in patients with symptomatic carotid stenosis.&lt;br /&gt;&lt;br /&gt;Google:  I searched the term "Endarterectomy, Carotid, Stenosis, Stroke" with sites only from .edu domains.  The first article, "Carotid Endarterectomy Update" from neurosurgery.mgh.harvard.edu/cea.htm, had information from relevant studies using patients with moderate carotid stenosis. &lt;br /&gt;&lt;br /&gt;CCHS Library:  I searched the term "Endarterectomy".  I then clicked on Ischemic Stroke Clinical Resources.  The only relevant document I found was a review on neurosurgery.  The article was from MDConsult.&lt;br /&gt;&lt;br /&gt;Search 3.   Clinical Prediction Rules&lt;br /&gt;&lt;br /&gt;Pubmed: I searched the term  "Ankle Injuries"[MeSH] AND "Ankle Injuries/radiography"[MeSH] AND "Guidelines"[MeSH].  I used English-only and Human as my limits.  I received 19 articles, most of which deal with the validation of the Ottawa ankle rules.  The article "Comparison of Ottawa ankle rules and current local guidelines for use of radiography in acute ankle injuries" uses a study to determine if the Ottawa ankle rules are the most efficient guidelines to observe when dealing with radiography of the extremities.   &lt;br /&gt;&lt;br /&gt;Google:  I searched the term " Ottawa ankle rules" and chose the article labeled "Cost-effectiveness analysis of the Ottawa Ankle Rules".  This article provided a summary of the decision rules and a section on the cost of x-rays.&lt;br /&gt;&lt;br /&gt;CCHS Library:  I searched the term "ankle injuries" and chose the ankle fracture clinical resources link.  I then chose a Diagnostic Strategies link from MD Consult.  The article provided information on the efficiency of using the Ottawa ankle rules in order to diagnosis ankle fractures. I then clicked on Related Links where I was provided several articles on the use of warfarin and other anticoagulants in the treatment of congestive heart failure and atrial fibrillation.&lt;br /&gt;&lt;br /&gt;Google: I searched the term "anticoagulant therapy and congestive heart failure and atrial fibrillation" with the exact phrase "risk of hemorrhage". It returned about 160 articles&lt;br /&gt;I chose an article found at &lt;br /&gt;stroke.ahajournals.org/cgi/content/full/31/9/2266 &lt;br /&gt;There were no similar pages.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;CCHS Library: I searched the term "warfarin" and chose the heading "Anticoagulant, Fibrinolytic and Antiplatelet Therapy Clinical Resources". I searched through the various documents and found "Antithrombotic and Thrombolytic Therapy in Cardiovascular Disease".&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 2 Screening&lt;br /&gt;&lt;br /&gt;Pubmed: I searched the term "Colorectal Neoplasms"[MeSH] AND "Colorectal Neoplasms/prevention and control"[MeSH] AND "Mass Screening"[MeSH] AND "Mass Screening/standards"[MeSH]. I then clicked on the article with the title "Colorectal cancer screening: clinical applications." I received 32 returns.  The article "Advances in osteoporosis therapy. 2003 update of practical guidelines" was one of few that seemed pertinent.  I clicked on related links and did not see anything with practice guidelines.  I attempted to limit my search to practice guideline in the publication field but did not receive any results.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Google: I searched the term 'hormone replacement therapy practice guideline" and received about 40000 results.  I clicked on "ASRM: Menopause, Hormone Replacement Therapy (HRT), Osteoporosis".   This information provides several guidelines for hormone replacement.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;CCHS Library: I searched the term "hormone replacement therapy" and received 5 results.  I clicked on "Menopause Clinical Resources".  At the top of the page I clicked on clinical guidelines.  I then searched the page titled "Postmenopausal Hormone Prophylaxis: Access document".  This provided an enormous amount of information on the clinical guidelines and current practices of using hormone replacement therapy in menopausal women.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 2  Variations in Outcomes of Health Services&lt;br /&gt;&lt;br /&gt;Pubmed:I searched prostatic hypertrophy in MeSH and it returned prostatic hyperplasia.  I then searched the following terms "Prostatic Hyperplasia"[MeSH] AND ("Prostatic Hyperplasia/mortality"[MeSH] OR "Prostatic Hyperplasia/surgery"[MeSH]".  The return was large but I picked the article labeled "Long-term follow-up after transurethral resection of the prostate, contact laser prostatectomy, and electrovaporization".  &lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Google:  I searched the term "transurethral prostate surgery" for articles in English and that had been updated within the last year.  There were 42 returns.  I chose the article from "urology.jhu.edu/diseases/prostate", which is from Johns Hopkins.  It talks about the complications from the procedure and compares it with other alternatives.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;CCHS Library: I searched the term "transurethral prostate surgery" and received two returns.  I chose the resource "Benign Prostatic Hyperplasia Clinical Resources".  After I clicked on this link, I was directed to a host of resources.  I chose the article labeled "Assessing The Effectiveness And Safety Of Medical Therapy For Benign Prostatic Hyperplasia", which provided a lot of information about the procedures for treating BPH.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 3.   Clinical Utilization Review&lt;br /&gt;&lt;br /&gt;Pubmed: I searched "Angioplasty, Transluminal, Percutaneous Coronary"[MeSH] AND "Angioplasty, Transluminal, Percutaneous Coronary/utilization"[MeSH]" and it returned 285 articles.  I clicked on the related links icon next to the article "Summaries for patients. Overuse of coronary artery bypass graft surgery and percutaneous transluminal coronary angioplasty."  This article provided some information on overuse of certain procedures.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Google: I searched "Angioplasty, Transluminal, Percutaneous Coronary" and then read the article "The appropriateness of use of percutaneous transluminal coronary".  I clicked on similar pages and received 143 articles that were in the Pubmed database.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;CCHS Library:  I searched the term " Transluminal Percutaneous Coronary Angioplasty" and it returned to resouces.  I clicked on the resource labeled "Cardiac Catheterization Clinical Resources", which directed me to Clinical Guidelines from the American Heart Association.  The access documents provide information in to what is the appropriate situation for using the procedure.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Tim Prestley&lt;br /&gt;Feb. 18&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 1. Prognosis was not a subheading, so I searched it as a separate MeSH heading. I then searched Pubmed with Alzheimers Disease AND Alzheimers/ mortality AND Prognosis in the search field with only English and only title/abstract limits. This returned 31 results, of which are about life expectancy.&lt;br /&gt;&lt;br /&gt;Google search for Alzheimers Prognosis and Mortality: I typed Alzheimer's Prognosis Mortality into the search field and set only English as limit in the advanced search field. This returned 5020 results. &lt;br /&gt;&lt;br /&gt;CCHS Library search for Alzheimer's Prognosis and Mortality:This did not provided any results. I then searched Alzheimer's. This returned 7 results. I opened the first result which provided links to several sites. I clicked on results from MDConsult. The Alzheimer's disease link from MDConsult provided information on several aspects of Alzheimer's.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 2. Overview&lt;br /&gt;Pubmed search for Hypercholesterolemia AND Hypercholesterolemia/ diet therapy: I searched this using MeSH headings and subheadings. I used meta-analysis and English only as limits. The search provided 8 results. &lt;br /&gt;&lt;br /&gt;Google search for Hypercholesterolemia and diet therapy: I entered the terms "hypercholesterolemia diet therapy" in the advanced search field "all words. I used English only as my limit. The search returned 27200 results.&lt;br /&gt;&lt;br /&gt;CCHS library: I searched the keyword hypercholesterolemia. 2 results were found. I clicked on the hyperlipidemia clinical resources link. MDConsult had an access document on the lipid lowering trials using diet therapy.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 3. Clinical Decision Analysis&lt;br /&gt;&lt;br /&gt;Pubmed: I entered congestive heart failure into the MeSh search field. I sent the best result to Pubmed search field. I then searched warfarin. I sent the best result to the Pubmed search field. I searched "congestive heart failure" AND " warfarin". No results were found. I then searched Warfarin/therapeutic use. This returned 32 results, many on the use of warfarin in patients with A-fib. There were a few clinical trial results, but I don't know if they are pertinent to this case. &lt;br /&gt;&lt;br /&gt;Google search: I entered "congestive heart failure warfarin" into the advanced all words field. This returned 5020 results.  &lt;br /&gt;&lt;br /&gt;CCHS Library search for Alzheimer's Prognosis and Mortality:This did not provided any results.  I then searched Alzheimer's.  This returned 7 results.  I opened the first result which provided links to several sites.  I clicked on results from MDConsult.  The Alzheimer's disease link from MDConsult provided information on several aspects of Alzheimer's.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 2.  Overview&lt;br /&gt;&lt;br /&gt;Pubmed search for Hypercholesterolemia AND Hypercholesterolemia/ diet therapy:  I searched this using MeSH headings and subheadings.  I used meta-analysis and English only as limits.  The search provided 8 results.  &lt;br /&gt;&lt;br /&gt;Google search for Hypercholesterolemia and diet therapy: I entered the terms "hypercholesterolemia diet therapy" in the advanced search field "all words.  I used English only as my limit.  The search returned 27200 results.&lt;br /&gt;&lt;br /&gt;CCHS library:  I searched the keyword hypercholesterolemia.  2 results were found.  I clicked on the hyperlipidemia clinical resources link.  MDConsult had an access document on the lipid lowering trials using diet therapy.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Search 3.  Clinical Decision Analysis&lt;br /&gt;&lt;br /&gt;Pubmed:  I entered congestive heart failure into the MeSh search field.  I sent the best result to Pubmed search field.  I then searched warfarin.  I sent the best result to the Pubmed search field.  I searched "congestive heart failure" AND " warfarin".  No results were found.  I then searched Warfarin/therapeutic use.  This returned 32 results, many on the use of warfarin in patients with A-fib.  There were a few clinical trial results, but I don't know if they are pertinent to this case. &lt;br /&gt;&lt;br /&gt;Google search:  I entered "congestive heart failure warfarin" into the advanced all words field.  There were 20600 results.  Many were studies that investigated the use of warfarin with congestive heart failure patients.&lt;br /&gt;&lt;br /&gt;CCHS library:  I searched the library using the phrase " congestive heart failure drugs".  There were 5 returns that were viewable.  I chose the Dilated Cardiomyopathy Clinical Resources heading.
